HIGH SCHOOL FORMS

 

 

FHSAA requires that a student-athlete be completely registered at athleticclearance.com before he/she can participate in any type of preseason or offseason conditioning, workouts, practices and contests.

  EL2 Feb 2026 update

Pre-Participation Physical Evaluation -  This form must be printed, completed by a physician, and either uploaded to athleticclearance.com or submitted to your school's Athletic Director.

* NEW FHSAA EL1 form (ECG)

The following forms must be completed during the online registration process through athleticclearance.com.       

2026-27 Annual Drug Consent form (required by Escambia County School District)(1 page)

ECPS Code of Conduct/Sportsmanship Form

MIDDLE SCHOOL FORMS

The Escambia County School District requires a student-athlete to complete all documents before he/she can participate in any kind of conditioning, workouts, practices and contests.

MS Pre Participation Physical Evaluation (2 pages)

MS Consent and Release from Liability Certificate (1 page)

2026-27 Drug consent form (required by Escambia County School District)(1 page)

MS Code of Conduct/Sportsmanship Form

All above mentioned forms can be provided by all schools.
